Terms of Electronic Service Provision

§ 1. General Provisions

These Terms and Conditions define the rules and procedures for booking and scheduling appointments through the website: https://nowaclinic.pl.

Definitions:

  • Price List – the list of medical services and their prices made available on the Clinic’s website.
  • Patient – a natural person using the services provided electronically by the Clinic.
  • Regulations – this document, available on the Clinic’s website in a form that allows it to be saved and printed.
  • Website – the Clinic’s website available at https://nowaclinic.pl.
  • Service – an electronic service enabling the Patient to book or schedule an appointment online.
  • Service Provider / AdministratorNOWA Sp. z o.o., headquartered in Warsaw (02-707), ul. Puławska 111A/5/3, registered in KRS under number 0001048792, NIP 7011157594.

In order to use the Service, the Patient must read and accept these Terms and Conditions.
Failure to accept them makes it impossible to book an appointment.

§ 2. Use of Services and Payments

Appointments can be booked via the registration form available in the “Book a Visit” section of the Website.

The Patient is required to:

  • provide accurate personal data (name, surname, phone number, and e-mail address),
  • make a prepayment of PLN 100 when booking appointments lasting 60 minutes or longer.

If the prepayment is not made, the appointment will be automatically cancelled 48 hours before the scheduled time.
If the Patient cancels the visit more than 24 hours in advance, the prepayment remains as a deposit to be used later.
If the Clinic cancels the visit, the deposit also remains available to the Patient.

§ 3. Complaints

The Patient has the right to submit complaints regarding the Service via e-mail to the address provided above.
Complaints will be processed within 14 working days from the date of receipt.

§ 4. Cancellation or Rescheduling

Appointments can be cancelled no later than 24 hours before the scheduled time.
Failure to attend or cancel within the required timeframe results in the forfeiture of the prepayment, regardless of the reason.

Late arrival will shorten the duration of the visit by the amount of the delay, while the price remains unchanged.
If the delay exceeds one quarter of the scheduled time, the Clinic reserves the right to cancel the appointment, and the prepayment will not be refunded.

The Clinic may cancel a visit up to one hour before the scheduled time, offering a new appointment.
If the Patient declines the new date, the prepayment remains as a deposit.

§ 5. Technical Requirements

To access the Website and booking form, the Patient must use an Internet-connected device with one of the following browsers: Chrome, Firefox, Edge, Opera, or Safari.
An active e-mail address and the ability to make online payments are also required.

§ 6. Final Provisions

The Clinic reserves the right to amend these Terms and Conditions at any time.
Any disputes between the Clinic and the Patient shall first be resolved amicably.
If this proves impossible, the case will be handled by the competent court having jurisdiction over the Clinic’s registered office.

These Terms and Conditions enter into force on January 2, 2025.
